If you are with a healthcare institution interested in purchasing AMT products for the first time, please submit the new account application via fax or email. Download the new account application by clicking on the icon above.

2. CHOOSE PAYMENT METHOD

AMT accepts payment via NET 30 terms or prepayment/credit card.

To establish terms, please fill out an AMT Credit Application and submit via fax or email. Three solid references must be included. Download the new account application by clicking on the icon above.

All orders are accepted via fax or email.
If paying by credit card, payment information can be directly called in.

3. PLACE YOUR ORDER

Regular Orders: Orders for items that are in stock will ship the next business day.

Expedited Orders:  Orders for items in stock and received before 12:00 PM EST will be shipped the same day.

All orders shipped via UPS FOB Origin. No third-party shipping.
